The way I understand this Womans maiden has reference to the officiant and that is myself my maiden name was

Divorced 8 day of Sept 1931 at Fredonia, KS
Married to  Lemmuel E. Riley August 3 1936 at Alamosa [CO?]
Mr. Riley passed away at that place Sept 10 1936.
Married again Dec 22, 1948 at Denver.
His name is Gates Mr. Frank Gates. 
He left here May 5, 1949 no support since.
Mr Riley is the only one that had been married before he married me and I have no record of his wife.
Not[h?]ing Pre[ceding?] 10. but as I haven't noted since my name was changed as we have no noting yet this year my [underscored] name will b e Riley.
but the address will be the same as us[u]al. I have letters and cards to show I have been at this address since 1944.
